Wellness Stacks

Small daily steps that drive the whole operation.

Six Wellness Stacks give each person simple things to do every day. Every check-in, goal and milestone flows back to your team as signals, trends and the data your funders and surveyors ask for — on the same record.

Wellness apps stop at the participant. Records stop at discharge.

Wellness tools usually live in a separate app your team never sees, and the record usually ends when the episode of care does. Wellness Stacks connect the two: what a person does each day becomes something your organization can see, support and report.

The six stacks

Six stacks. One person.

Each stack offers a few small, doable things a day. People earn points and milestones as they go, and see their own progress.

Mental

Breathing resets, mood check-ins and words for how the day is going.

e.g. 5-minute breathing reset

Physical

Movement that’s safe for where someone is in their recovery.

e.g. Take a 20-minute walk

Nutritional

Simple, everyday habits that steady the body.

e.g. Drink 8 glasses of water

Sleep

Wind-down routines and sleep meditation.

e.g. Screens off by 10 PM

Spiritual

Meaning, gratitude and connection, in the person’s own terms.

e.g. A gratitude practice

Financial

Budgeting basics and money stress, as part of staying well.

e.g. Set a weekly budget

How the stacks drive the engine

From one person’s morning to your quarterly report.

The stacks aren’t a separate wellness app. They’re the front of the same record your team works in, so every small step adds to what you know.

  1. They act

A daily check-in, a suggestion from each stack, a goal they set themselves.

  2. It becomes signal

Mood, cravings, sleep, streaks and milestones, captured as they happen.

  3. Your team sees it

The same person’s check-ins, trends and milestones in the staff app, beside their programs and notes.

  4. HOPE personalizes

HOPE coaches around what the person is actually working on that week.

  5. You report it

Standardized assessments and trends roll up into dashboards and the reports funders and surveyors ask for.

When someone’s signals change, early-warning alerts tell your team before the next appointment.

Progress people can see

Goals, points and milestones that keep people coming back.

  • Goals they set themselves, tagged to a stack
  • Points for each small step, and streaks for showing up
  • Milestones like “Fifteen Days of Consistency,” celebrated in the app
